DIRECTIONS to Submit Required Athlete Paperwork
St. John Paul the Great Athletics uses a software called Athletic Trainer System (ATS) to enhance sports medicine care and communication between the athletic trainers, parents, athletes, and to show coaches “cleared to play” status. Please use this site to upload your completed Virginia High School League (VHSL) sports physical and complete your Injury & concussion consent form electronically. Do not turn in paper copies to coaches, athletic director, or JP front office staff.
Instructions:
1. Using a web browser, go to greatcare2.atsusers.com to log in.
Athlete ID = assigned Student ID Number (7 digits number)
Initial password= Welcome1 for freshmen and new student transfers.
You will immediately be asked to update the password. Make it memorable & very strong. Once you update the password, save it because you will use it to upload VHSL physical every school year. Parents and students should pick a password they can both remember. There is no automated way to reset passwords.
2. Click on “Athlete Information” to review your record/ profile details
● Parents, you are accessing your child’s ATHLETIC health record (functionality similar to MyChart)
● Add the JP student email and gender if they are blank. A school appropriate photo can be uploaded if you wish. Parent contact info gets listed as “Emergency Contact”.
● Verify, or complete other highlighted boxes. Select “save” at the bottom to save edits. Select “verify” if nothing is altered.
3. Under “Efiles” you can print out a blank VHSL sports physical form to take to your doctor’s appointment. After seeing the doctor, upload the completed VHSL sports physical document.
● Scan ALL 4 pages into a single pdf file. Name it “sports physical 2024”.
● There is a file size limit, so compress it, and make sure it’s readable.
● You may need to use a scanning app or adjust phone camera settings to save images as a pdf.
4. Under “Forms,” fill out the required “Injury & Concussion Acknowledgement Consent” with a parent/ guardian.
● Select the form from the drop down menu.
● Select “new” to create a new one for 2024.
● Click the relevant answers in the electronic form, sign & select “Save” at the bottom.

NOTE ❖ Parents & athletes can see the status of their requirements under the “Paperwork” tab.
○ The Athletic Trainer (AT) will review & verify the sports physical once it is submitted.
○ The AT determines “cleared to play” status when both requirements are met and sends report to athlete's coach.
