Attention current Sophomores who will be Juniors next school year (2025-2026):

There is an opportunity to get your ImPact Concussion Baseline Test done before the school year ends. 

NEXT AVAILABLE TESTING DATE:

Remember, this is a requirement for Juniors in order to play JP Sports. 

Before sitting for the concussion baseline test:

1. Student athlete and their parent/guardian complete the Injury & Concussion Acknowledgement Form and electronically sign the form on the Athletic Trainer System (ATS).

2. Go to PaySimple to pay the $30 fee for the ImPact Concussion Baseline Test. Bring a copy of the receipt to the testing session.
