Attention current Sophomores who will be Juniors next school year (2025-2026):

There is an opportunity to get your ImPact Concussion Baseline Test done before the school year ends.

Remember, this is a requirement for Juniors in order to play JP Sports. 

Before sitting for the concussion baseline test:

1. Student athlete and their parent/guardian complete the Injury & Concussion Acknowledgement Form and electronically sign the form on the Athletic Trainer System (ATS). Once logged in to the ATS portal, click on "Athlete Information" and then click on the "Forms" tab. On the drop down menu, find "Injury Concussion Acknowledgment Consent" and then click "New." Both athlete and parent must read and electronically sign the document and save to your ATS profile.

2. Go to PaySimple to pay the $30 fee for the ImPact Concussion Baseline Test. Bring a copy of the receipt to the testing session.
